Elamipretide, Mitochondria targeting tetrapeptide
A cell permeable tetrapeptide that binds cardiolipin in the inner mitochondrial membrane, stabilizing cristae and improving bioenergetics, the first drug to directly target mitochondria.
Mechanism of action
A cell permeable peptide that homes to the inner mitochondrial membrane and binds cardiolipin, stabilizing the energy machinery and cutting oxidative stress.
Also studied
Stabilizes cristae and improves bioenergetics by binding cardiolipin, the first drug to directly target mitochondria.
See related compoundsAccelerated FDA approval for Barth syndrome; the randomized phase missed its endpoints, with approval resting on open label knee extensor strength.

Key finding
In the open label extension, elamipretide was well tolerated with sustained improvements in functional assessments and cardiac function.
Full profile
FDA accelerated approval (Sept 2025) for Barth syndrome, the first drug to directly target mitochondria. The randomized crossover portion of TAZPOWER (12 subjects) did NOT meet its primary endpoints; approval rested on the open label extension showing improved knee extensor strength. Longevity/anti aging uses are entirely extrapolative.
Most common adverse event was injection site reactions (treatable). Continued approval is contingent on verifying clinical benefit.
FDA approved (rare disease, Stealth BioTherapeutics). In off label/wellness use it would be WADA prohibited under S0.
Daily subcutaneous (40 mg in trials).
