Mechanism of action
A heptapeptide derived from ACTH but stripped of its hormonal effect, used in Russia to raise BDNF and protect the brain, especially after stroke.

In 30 acute stroke patients given Semax plus intensive therapy vs 80 controls, Semax was associated with faster regression of cerebral and focal motor deficits (effect sizes not reported).

A registered prescription nootropic in Russia (since ~2011) used for stroke/cognitive impairment; Russian studies support neuroprotection, attention, and memory, especially post stroke. Trials are mostly small, single institution, Russian language, rarely blinded to Western standards, and largely absent from FDA databases.
Generally well tolerated in Russian use; long term and Western safety data are lacking.
